IMPORTANT INFORMATION REGARDING THE FUND’S MRI NETWORK

 

On Friday April 7, 2010 the Fund Office was notified that the MEDLINK MRI Network had suspended operations.  On a temporary basis, the Fund office has arranged for Members who are in need of MRI/Cat scan services to use the DiaTri National Diagnostic Imaging Network & Scheduling Service.  To use the DiaTri network please call 1-800-331-5720.  Let DiaTri’s staff know that you are a participant of the Midwest Operating Engineers Health & Welfare Fund and they will assist you in setting up an appointment for your MRI/CAT scan.  Your MRI/CAT SCAN BENEFIT HAS NOT CHANGED.  If you use the DiaTri Network, your services will be paid at 100% less any portion of your annual deductible that remains.

PINNACLE HEALTHCARE CENTER IN CROWN POINT INDIANA JOINS THE PPO NETWORK

Effective October 1, 2009, Pinnacle Healthcare Center located in Crown Point Indiana has joined the PPO network.  Members can now take advantage of this facility and receive in-network benefits.  This means that after satisfying your deductible, the Fund Office will cover 90% of all eligible expenses for services you receive at Pinnacle Healthcare Center.

Child Leaving College?

If your covered dependent child graduated college this spring, or if your child left college and is not planning to go back in the fall, his or her coverage under the Health and Welfare Fund will end as of June 30. If you would like to continue coverage for your child under COBRA, you or your child must notify the Fund Office, in writing, within 60 days of when your child would otherwise lose eligibility.

Need medical care while traveling?

If you need emergency medical care anywhere, go to the nearest facility for treatment. If you need non-emergency medical care while outside the Illinois area, call BCBS at 800.810.2583 to find a network provider near you or log on to BCBS.


Do you work out of the areA?

If you work outside of Local 150’s jurisdiction for another local, you may transfer contributions made to another local union pension plan to the MOE Pension Plan provided the other pension plan is a plan reciprocal to Local 150. Call your District Office or 708.579.6620 or click here to request a Transfer Authorization Form. After the Fund Office receives your Transfer Authorization Form, future contributions will be transferred from the other plan to the MOE Pension Plan. You can also transfer up to six months of contributions going back before you submitted the Transfer Authorization Form.


if you're retired and return to work, DID YOU NOTIFY THE FUND OFFICE?

If you’re retired, receiving benefits and you return to work, you must inform the Trustees in writing within 30 days of starting work. Certain work after retirement is considered disqualifying and can result in a suspension of your pension benefits. Before you reach normal retirement age, your benefits will be suspended for any month you work in disqualifying employment. After you reach normal retirement age, your benefits will be suspended in any month in which you work at least 41 hours in disqualifying employment.
